Health Care Coordinator

The Health Care and Quality Coordinator serves as liaison between program personnel, the delegating nurse and the health care team and is primarily responsible for documentation, taking residents to appointments, communicating to the programs, and working with direct staff in assisting with improved care.
Essential Duties and Responsibilities Providing the necessary care for individuals, including accompanying them to medical, dental, mental health, or other appointments as needed.
Scheduling and logging health care appointments.
Advocating for individuals when accompanying them to health care appointments.
Assisting the health care provider during medical examinations of individuals.
Implementing medical procedures as directed by nurse, health care provider or appropriate supervisor.
Ensuring that orders are clearly understood before leaving the health care provider's office and relaying this information to the supervisor in charge and/or to site staff.
Ensuring that follow-up appointments are scheduled and that all documents pertaining to the health care appointment are complete and filed appropriately.
Assisting with charting, filing and other record keeping tasks.
Posting orders for medications and treatments as prescribed.
If requested/directed, timely attendance and participation in meetings, including but not limited to:
staff meetings, clinical meetings and medication reviews.
Communicating all health care concerns in a timely manner to all appropriate persons.
Conducting inventory and maintenance of all medical and first aid supplies at all program sites.
Investigating and resolving all billing to individuals for health related services.
Ensuring that coverage is provided for all medical appointments and collect all necessary paperwork relevant to the appointment.
Disseminating all relevant medical information to appropriate staff, including but not limited to:
nursing reviews, Medical Appointment Records and PMOFs.
Acting as a liaison between Linwood and the delegating nurse(s).
Working on site in the homes and exploring ways to improve functions for the residents that will provide a safer and healthier environment.
Other Duties & ResponsibilitiesMay perform other duties as assigned.
Minimum Qualifications (Education, Experience, Skills) Must be at least 21 years old and have a high school diploma or its equivalent.
Must demonstrate the ability to interact in a professional manner and be able to represent Linwood.
Must be able to complete reports in a timely manner and must demonstrate the ability to organize and properly document that which is required by regulation and by Linwood.
Must also demonstrate an understanding of fundamental rights and self-determination and assure that the individuals to whom he/she provides support are afforded these rights.
Must obtain all trainings as required by regulatory agencies and Linwood within the time frames specified.
Must possess a skill set that includes the ability to interface in a professional manner with Linwood staff, parents, school system personnel and other professionals.
Must possess excellent oral, written and presentation skills.
Pay:
$21/hour Recommended Skills Billing Clinical Works Communication Filing First Aid And Preparedness Maintenance Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
